SmileFirst - Referral/Interest Form

The SmileFirst oral health education program provides interactive and informative presentations about oral health, dental hygiene, and preventing tooth decay in young children as well as the whole family.


Classes open to: MediCal members living in Santa Clara County who are parents/caregivers. Classes are available in English, Spanish, and Vietnamese, online on Zoom.

El programa educacional SmileFirst ofrece presentaciones interactivas e informativas sobre salud bucal, higiene dental y prevención de caries, tanto en niños pequeños como a toda la familia.


Clases disponibles para: Padres o cuidadores miembros de Medi-Cal que viven en el condado de Santa Clara. Las clases se imparten en inglés, español y vietnamita, y actualmente se ofrecen de forma virtual a través de Zoom.

Chương trình giáo dục sức khỏe răng miệng SmileFirst cung cấp các buổi chia sẻ mang tính tương tác và giàu thông tin về sức khỏe răng miệng, vệ sinh răng và cách phòng ngừa sâu răng cho trẻ nhỏ cũng như cả gia đình.

Các lớp học dành cho: Phụ huynh hoặc người chăm sóc trẻ là thành viên chương trình Medi-Cal và đang sinh sống tại Quận Santa Clara. Lớp học được tổ chức trực tuyến qua Zoom với các ngôn ngữ: tiếng Anh, tiếng Tây Ban Nha và tiếng Việt.

*Số lượng chỗ dành cho người không tham gia chương trình Medi-Cal có hạn.
